Submitted an extension for a client last week. He did not make any payment on initial extension; today is the last day he want to mail payment. What form should be used?

He is not yet ready to file taxes but just want to mail payment since he was able to find some money.

4686 can be filed with the payment, or if paying online can choose extension.

We originally submitted extension without payment. Can we submit extension twice using proseries basic?

You can try.  If PS doesn't allow a second extension, the easiest thing for your client to do is to go with Direct Pay.  If your client is old fashion, a check postdated today by USPS certified mail or an approved PDS will be deemed a timely payment even though the IRS may receive that only after the due date.
