Why doesn't doesn't gross 1099R'S amount equal line 4a on 1040 form?

According to the 1040 instructions, if the IRA is fully taxable the amount doesn't need to be shown in 4a.

I remember this from last year, if the gross and taxable are the same, nothing shows in the gross box...do things look different than last year or are people just super scrutinizing everything this year?

Pensions and IRAs were on two separate lines last year so things didn't have the funky appearance that it does this year.
