It seems only two items can be input on the Form It 212 worksheet. Any one else having difficulty?

Please forgive my tardiness in responding.  I have been completely under the weather for the last week or so.   It turned out that the client did not meet the qualifications so I did not have to complete the form.  Thank you for your input.
