Does a W2 with no income in box 1 need to be added to the tax return if the W2 is for home health care of a relative who lives with tax payer.

Does it have any codes in the box 12's. Are you sure there is only 1 page to the W2?
The W2 has no codes in box 12 and there is only one page of the W2 it is for home health care of a relative who lives with the tax payer.
