How to answer the question "did you have health coverage all year" yes or no? to avoid a no coverage penalty?

If the client didn't have health insurance, it's kind of tough to answer yes.  Just because they didn't have insurance doesn't automatically involve a penalty, but it could involve more work for you to determine is coverage was not affordable.
