I want to add a footnote / statement to a 1040 and don't see how.

You can't. The only custom statement you can prepare is for election but it will clearly say election on the top, so it won't meet your needs. Neither does ProConnect Tax have a function to produce custom footnotes.
For the statement, what we do is to attach that as a PDF to the return before e-filing.
