Good luck with that.  That form was "requested" in something like 1962 and one of the moderators at that time was going to "add it to the list of enhancements" for us.  I used to have a handful of those returns when the request was first mentioned in version 1 of this forum.  Most of those clients have died off so I think Intuit is waiting for my last client to kick the bucket before they announce that they have listened to their customers and have added the form.
