Where Do I put prior years (11 Years worth) Of loss carry forwards.

I know there is a Federal Carry forward worksheet for State and local taxes and below that are loss carry fwd for Short and Long term capital losses with adoption credit/ carry fwd

But where do I list the years and losses so it appears under Other Income on the 1040

This is a new client for me
