Having the same problem. Can't find any information on it either.

Same here too no way to figure out where this form is hidden.

I was able to figure out the way, here are the steps 

1) In federal - info worksheet in section VI - electronic filling in the states select New york city UBT or 1127

2) than go to NY State return and click on the file menu on the top and select Go to City/State return and you will able to get 1127.
