Sometimes we get a tax form without the letter that may have come with it. Here is information on a letter that came with aforementioned form and this may help someone with this form: "For tax qualified contracts: You should retain the enclosed IRS form 1099 LTC for your tax records, but , since your benefits are not taxable, you are not required to file any IRS forms or take any other action as the result of receiving the enclosed form 1099 LTC." Again,  This is for tax qualified contracts. Look on the form 1099LTC on box 4, and see if the qualified contract box is checked, but the box is marked optional. The letter also addresses "For contracts that are NOT tax qualified". So you may want to ask for the letter that came with the form if there was A LETTER. Hope this helps someone.
