Perfectly acceptable to use the grantor's SSN while the trust is still revocable.  This is a new client, you hadn't noticed it in previous years?  

You don't really need the trust document, you just need the part that names the successor trustee.  If it's the husband, are there kids or grandkids who can help him get help?
