Look for Screen 13.1 under Form 8915.  The last I checked earlier this week, F.8915-E can only be viewed as a reference copy but not finalized.

Since the form can't be e-filed, you'd need to attach it as a PDF; F.8915E is already listed in the dropdown for forms to attach.
