Use Screen 65.2, California Power of Attorney (3520-PIT)/Tax Information Authorization (3534), to generate Form 3520. Check the box labeled Print Form 3520-PIT with complete return and fill out the Representative information.

Lacerte doesn't print Business Entity Identification numbers, such as the California Corporation number, the Secretary of State number and Federal Employer Identification Number, on the Form 3520 in the Individual module. Likewise, Social Security numbers don't print in the Business modules.
