How to resolve Lacerte diagnostics ref. 4975, ref. 7936, ref. 7937, ref. 7938

This article will help you resolve diagnostics that appear when a filing address is missing on an Ohio generic city return. The diagnostics state:

For individual returns: "The filing address for an Ohio Generic City return must be entered. One or more generic returns are present without a filing address entry." (ref. #4975)

For S Corp or Partnership returns:

  • "An entry is required for the 'City, State, ZIP code' for all Generic cities." (ref. #7938)
  • "An entry is required for the 'Filing address' for all Generic cities." (ref. #7937)
  • "An entry is required for 'Filing addressee' for all Generic cities." (ref. #7936)

To resolve the diagnostics

  1. Go to Screen 54, Taxes under the State & Local section.
  2. Select Ohio Cities (Screen 54.123) under St. Taxes from the left navigation panel.
  3. Click on '1' or add a city under the City list box.
  4. Select the city in Name of city (Ctrl + T) (code 1).
  5. Under the Filing Address [O] section, enter the following information:
    • Addressee (code 29),
    • Street (code 30), and
    • City, State, ZIP code (code 31).
  6. Provide the mailing address that appears on the filing instructions and slip sheets.
  7. Once the information is entered, the diagnostic is removed.
  1. Go to Screen 48, Taxes under the State & Local section.
  2. Select Ohio Cities Apportionment, Misc., Adjustments (Screen 48.121) from the left navigation panel.
  3. Click on '1' or add a city under the City list box.
  4. Select the city in Name of city (Ctrl + T) (code 1).
  5. Under the Filing Address (Mandatory For Generic) section enter the following information:
    • Addressee (code 76),
    • Street (code 77), and
    • City, State, ZIP (code 78)
  1. Go to Screen 43, Other Forms under the State & Local section.
  2. Select Ohio Cities (Screen 43.129) from the left navigation panel.
  3. Click on '1' or add a city under the City list box.
  4. Select the city in Name of city (Ctrl + T) (code 1).
  5. Under the Filing Address (Mandatory For Generic) section enter the following information:
    • Addressee (code 76),
    • Street (code 77), and
    • City, State, ZIP (code 78)

For all returns

  1. Provide the mailing address that appears on the filing instructions and slip sheets.
  2. Once the information is entered, the diagnostic is removed.
  3. Entries are mandatory for those cities using the generic return. See ohio.gov for a list of city addresses.
