Entering Form 5498-SA in ProSeries

ProSeries does not have a dedicated 5498-SA worksheet. Information from the 5498-SA may be needed to complete Form 8853, Archer MSA's and Long-term Care Insurance Contracts or Form 8889, Health Savings Accounts (HSAs).

For Medical Savings Account MSA:

  1. If the MSA Contributions were made through the employer, they should be shown on the W-2 Worksheet, box 12 with a code R. Make sure these account for the contributions on the 5498-SA.
  2. If the MSA Contributions we're made by the taxpayer/spouse outside their employer:
    1. Press F6 to bring up Open Forms.
    2. Type 8853 and select OK.
    3. Scroll down to the Part 1 Smart Worksheet.
    4. Select the QuickZoom to limitations worksheet.
    5. Complete the MSA information and enter the contributions on line 1.

For Health Savings Account HSA:

  1. If the HSA Contributions were made through the employer, they should be shown on the W-2 Worksheet, box 12 with a code W. Make sure these account for the contributions on the 5498-SA.
  2. If the HSA Contributions we're made by the taxpayer/spouse outside their employer:
    1. Press F6 to bring up Open Forms.
    2. Type 8889 and select OK.
    3. In the Line 3 Smart Worksheet fill in the information about the type of plan.
    4. Enter the contributions on line 2.
