You enter the info from the F 3895 in the same place you entered info from the F 1095-A. And you have to have the F 3895, as the numbers are not always the same as the F 1095-A. Your client will have to get the form for you.
Doing so generates the F 3853

Yes, Covered CA in their infinite wisdom, sent these 2 forms 1095A and 3895 in two separate envelopes with a bunch of other junk paperwork included making it more difficult to locate the necessary form.
