medicare paid thru pension

client has Medicare premium take out thru monthly pension, spouse receives SSI and premium is deducted from their payment.  The system only allows the SSI premium and not the non SSI taxpayer's premium.  Where do I enter the Medicare premium paid from the pension.  It doesn't show on 1099R

Just plug them in here, IRS doesnt get this worksheet, its just for your records...you can add a description to it if you'd like, so you know what it is if you have to refer back later.  I think Line c only pulls from the SSA1099, which you don't have for this situation.
