I have a client that has a HSA that gross distribution is only 40.00 Do I need to fill out a HSA Form?

Yes, you will want to fill out the 1099-SA form in the program, I see youre using Basic, Im not that familiar with its features, but you should be able to access a list of forms to find the 1099SA
