I have a short term capital gain for a return that does not come from a 1099 B. It has not been reported to the IRS. How do I enter that on form 1040. I haven't been able to access any input form for anything other than 1099 B.

Agreed! It shouldn't be called the 1099B worksheet, it should be labeled Capital Transactions Worksheet.

Use the 1099B worksheet, just scroll down past the brokerage info and enter directly on the Sch D in the entry table provided.
