Help with form 8889

I'm trying to complete form 8889, but the IRS instructions are confusing. I don't think anything should be taxable because all distributions were used for medical expenses. Do I enter the contributions on line 2 of the form? All other info seems to flow correctly, but I'm seeing $400 or so as taxable on Sch 1. Please help!

Did you complete the 1099-SA worksheet and mark the box that the distributions were eligible?

I only see small worksheets within the form. Is there another place to look for it?

When you have clients open, click on the green box at top, where do I enter. Type in 1099SA. Under line 3 on the worksheet is a box to check that you used the full amount for medical
