IRS has no LLC Form.  Federally, LLC is either taxed as Partnership (yes, 1065) or Corp (1120), UNLESS it's SMLLC when it goes on 1040 Sched C.

"UNLESS it's SMLLC when it goes on 1040 Sched C." or E or F, or 1120 or 1120S if electing to be taxed as such.
