Client can't sign esignature

Hello.

I have a client I sent the 8879-S to. He enters all the same info as always in Docusign and its says info is invalid (i.e. address, etc.) He's tried spelling everything out, abbreviating, his social...nothing is working. I keep needing to void and resend. Now ProSeries has charged me 2 signatures (it's a corp so only one is required) and he hasn't even been able to send me back the 8879-S.

Any ideas?

PS any idea how to get an unused signature credited back to your account?
