Can a customer who is an American Citizen to avoid the fee which has been imposed for not being covered for medical insurance on 2018 ?

I have a client that claims he does not need to pay the fee imposed for not having medical insurance. He is a citizen, and did not have medical insurance for himself, for all of tax year 2018.

Is there any circumstance, where he does not need to pay this fee?

Are you kidding? there are at least a dozen exceptions to that penalty.  Review the instructions to Form 8965 for a list:

That statement seems like a 'red flag' to me, unless he told you WHY did does not need to pay the fee.
