Can a Taxpayer Deduct Household employee expenses for care of his wife that is critically ill ?

I have taken household employee expenses for a client that was a celebrity as miscellaneous expense. This is different however, because it is a caretaker. I was first thinking dependent care credit but this is the spouse. I also tried Head of Household but they are married.  Any suggestions?

No wonder the Hollywood elite hated the new Tax Cut....they already had that Celebrity Tax Credit....who knew???

Perhaps some of the expense qualifies as medical expenses.
