Failure for IRS to take out payments.

Form 4868 payments not taken out for Fed and state.  Installment agreement payments not taken out by IRS.  Reason for delay.  Here it is July 20, 2019 and payments were to  be taken out March 27, 2019.  All for 2018 tax return.

This sounds like a very good question for IRS. They are slow to (a) accept the offer and (b) begin to process payment.
