If I understand correctly.... When you go to TOOLS and BILLING and select RATES PER FORM, you do see amounts listed there?  Just not when viewing the actual invoice, is that right?

If you don't see anything in rates per form, check out your global billing options by going to TOOLS then BILLING then SET BILLING then GLOBAL BILLING OPTIONS, is the checkbox for "per form charges" checked?
