I have seen instances where a name/PTIN mismatch was done when registration was complete so that when the CE credit was sent to IRS, it failed that validation.  That may be the case for you, can you forward your proof of completion over to ProCustomerService@intuit.com and I can see if there is anything i can do.  (Assuming your CE credited course was done through Intuit).
